Case Management

Case Managers assists clients in navigating a dispersed healthcare environment. They serve as the referrer and service coordinator while advocating for you every step of the way. Services are offered in English and Spanish.

Case Management Services Include:

What type of services are provided?

Ongoing Case Management services to address areas of need for both individuals and family members impacted by a substance use disorder.

  • Referrals to treatment services when appropriate.

  • Assistance with insurance applications (Medical Assistance, Healthcare Marketplace, Pennie).

  • Assistance with housing referrals, recovery houses, and housing resources.

  • Assistance with SNAP Applications and food bank resources.

  • Referrals to mental health providers and primary care physicians.

  • Assistance with transportation to treatment and other emergent non-treatment needs.

  • Assistance with setting up utilities/utility bill assistance.

  • Assistance with enrollment in GED courses, college, financial aid applications, and English and a Second Language (ESL) courses.

DID YOU KNOW…

In data collected in 2021 (data.pa.gov), Black Pennsylvanians have the highest rate of fatal overdoses (6.8 per every 10,000 individuals), followed by Hispanic Pennsylvanians (4.3 fatal overdoses for every 10,000 individuals), with the fewest numbers of fatal overdoses occurring among Caucasian Pennsylvanians (3.7 per every 10,000 individuals). In Lehigh County, 19% of all fatal overdoses in 2021 occurred among Hispanic residents of the county and 6% of all overdose deaths were among Black residents of Lehigh County (overdosefreepa.org). That same year, Northampton County exhibited a higher rate of overdose deaths among Black residents (9%) than are represented in their overall population (5%) and reported 8% of overdose deaths among its Hispanic residents (overdosefreepa.org). We at MARS understand the need and are here to offer services to our community.

Services
The BIPOC Case Management Program focuses on the overall wellness of individuals by connecting them with a variety of services, programs, and community resources. It accomplishes this through advocacy, linkages, referrals, personalized community engagement, and by providing a supportive environment during recovery and sustained sobriety.

We offer the following BIPOC Case Management Services:

  • Referrals to treatment services, when appropriate

  • Referrals to Medication Assisted Treatment (MAT) Programs

  • Healthy Eating Program (Provided by the Kellyn Foundation)

  • Warm Hand-Off Services for clients reentering the community upon release from incarceration

  • Community Engagement

  • Connection to 12-step meetings/support groups

  • Referrals to Certified Recovery Support Services

  • Housing Assistance

  • Employment Assistance

  • Assistance with Navigating Healthcare Services and Needs

  • Transportation Assistance (To treatment programs)
