The HOST Program services all Lehigh County hospitals and is designed to connect patients with an identified Substance Use Disorder (SUD) to treatment. The HOST Program assesses patients, recommends a level of care, and coordinates placement from the hospital to Detoxification, Inpatient Residential, Outpatient treatment services, and Medication Assisted Treatment (MAT). If the HOST Program is unable to arrange a Warm Hand-Off, HOST will continue with placement from the community. Referrals can be made for all patients, regardless of insurance status. The HOST Program will work with the appropriate Single County Authority (SCA) to secure funding for treatment.
